Fit Note Requests
- GPs will generally accomodate fitnote requests, but there are legal requirements in order for them to do so.- Information regarding your diagnosis, how this affects your work and when you expect to return to work will be required.- It may not be possible for your GP to issue a fit note without speaking to you and you may be asked to make an appointment.- Your GP might also issue you with a note covering a shorter period than requested if you are invited for an appointment.- Your GP cannot issue a fit note / sick line for something they are unable to verify.- If you have been advised to stay off work due to a hospital visit or admission, your specialist team would usually issue the note.- Fit notes can be backdated, so although employers can be pushy, there is usually no urgency in issuing a fitnote.
Prescription requests
- This service is an alternative to calling the practice for a prescription.- The form helps us to proces your request faster and reduces the rates of errors from mispronounciations.- It is important to spell the name of your medicine correctly so it may be helpful to have the box nearby.- You can type multiple different medications into the request box.- It is helpful to write each medication on a new line (press enter / return after each medication).
Practice Feedback (Compliments)
- This form allows you to provide feedback in the form of a compliment.- We appreciate positive feedback, as well as negative, as it lets us know what is working well and shouldn't be changed.- We will pass on your positive feedback to the relevent member of staff or the wider team if applicable.
Practice Feedback (Complaints)
- This form allows you to provide feedback in the form of a written complaint.- We appreciate all feedback, positive or negative, as it lets us know what is working well and what is not working.- In order to identify areas where improvement is necessary, we need all the information on the complaints form.- We will let you know within 30 days whether your complaint has been upheld.- If you are a family member, please note we can only investigate issues with the consent of the affected individual.
Subject Access Request (SAR)
- You are able to request a copy of your medical record if required.- To do so simply complete the SAR form with all required information.- The practice will respond to your request within 30 days.- It is easier and faster for us to provide a record between specific dates if you know which timeframe is required.- You can receive your records either via an email or by providing us with a USB stick.- Note that the security / safety of your data if emailed is dependent on your email provider.
